HomeFront Haven Renewal Retreat Participant Consent, Assumption of Risk & Liability Waiver (Fiscally Sponsored by Veterans Collaborative)

We are honored and genuinely excited that you have chosen to participate in a HomeFront Haven Renewal Retreat. Your willingness to invest in your own well-being, growth, and connection speaks volumes. This retreat is designed to provide community-based mental health support in a space where military spouses feel seen, supported, and connected.

HomeFront Haven operates under fiscal sponsorship through Veterans Collaborative, a 501(c)(3) nonprofit organization. As such, the following Event Participation & Liability Release Notice applies to all HomeFront Haven programming, events, retreats, and initiatives.

1. Assumption of Risk

Participation in events and activities may involve inherent risks, including but not limited to physical injury, illness, emotional distress, property damage, travel-related risks, environmental conditions, interaction with other participants, equipment use, or third-party venue risks. You voluntarily assume all such risks associated with participation.

2. Release of Liability

To the fullest extent permitted by law, you release, waive, discharge, and hold harmless Veterans Collaborative, HomeFront Haven, and their affiliated initiatives, partners, officers, directors, employees, volunteers, contractors, or agents (collectively, the “Released Parties”) from any and

all claims, liabilities, damages, losses, or expenses arising out of or related to your participation, including claims based on negligence. This release applies whether participation is in-person, virtual, paid, unpaid, or volunteer-based.

3. Indemnification

You agree to indemnify and hold harmless the Released Parties from any claims or liabilities arising from your conduct during participation.

4. Medical Responsibility

You acknowledge that you are responsible for your own medical care and expenses and will inform organizers of any relevant medical conditions or limitations. In the event of an emergency, you authorize event organizers to seek emergency medical assistance on your behalf if necessary.

HomeFront Haven programming is supportive in nature and does not constitute medical or clinical mental health treatment.

5. Photo, Video & Media Release

By attending or participating, you grant Veterans Collaborative and its affiliated initiatives (including HomeFront Haven) the irrevocable right to photograph, record, and use your name, image, likeness, voice, and statements for lawful promotional, educational, reporting, fundraising, or marketing purposes in any media format without compensation. If you do not wish to be photographed or recorded, you must notify the event organizer in writing prior to the event.

6. Program Participation & Evaluation Commitment

HomeFront Haven Renewal Retreat participation includes engagement in the full continuum of support known as the “Steady Year” model. By enrolling in the program, you agree to actively participate in the following components:

• Completion of a pre-retreat survey

• Completion of a during-retreat feedback survey

• Completion of a post-retreat survey

• Participation in follow-up surveys at 3, 6, 9, and 12 months

• Engagement in scheduled check-ins with your assigned Anchor Guide

  
  

  Participation in designated touchpoints with HomeFront Haven staff or wellness leadership as outlined in program materials. These touchpoints may include virtual or written check-ins, community calls, or individual connection opportunities designed to support continued growth, belonging, and skill integration.

HomeFront Haven is a relationship-based program. Continued engagement helps ensure you receive the intended benefits of the experience and allows us to measure long-term impact in areas such as anxiety management, sense of belonging, resilience, and overall well-being.

Survey responses and personal information will remain confidential and reported in aggregate form unless explicit permission is granted.

Completion of these components is considered part of full program participation.

7. Condition of Participation

Participation in any Veterans Collaborative or HomeFront Haven event or activity constitutes acceptance of this waiver and release.

If you do not agree to these terms, you should not attend or participate.
